Surrogate maternity is an alternative method to treat infertility. Embryos retrieved during the cycle of IVF are transferred into the uterus of a woman who is not genetically connected with the embryo. The oocyte of an infertile woman is fertilized with her husband’s sperm and the embryo is transferred to the uterus of a surrogate mother.

Who can use surrogates

One of the most common causes to use surrogates is absent uterus. It can be a congenital pathology, Rokitansky–Küster syndrome, in particular. The uterus may be removed surgically to treat life-threatening conditions such as copious bleeding after C-section. There are other groups of patients who may be indicated for this assisted reproductive technology. For example, women with a high risk of miscarriage; patients with contraindications to pregnancy and labor. Among such diseases is heart failure. Women who had several unsuccessful IVF attempts may also choose such method as surrogacy.

Surrogacy process

This method uses the same principles as in cycles with donor oocyte. A woman who gives her oocyte, a biological mother, undergoes IVF treatment. A potential surrogate mother undergoes a procedure preparing her for the implantation. This procedure involves treatment with estrogens and progesterone.

Oocyte retrieval is quite easy procedure which is provided in an ambulatory unit using light anesthesia medications. Retrieved oocytes are fertilized and cultivated for several days. On the scheduled day, good quality embryos with high implantation potential are transferred to the uterus of the surrogate mother. The number of transferred embryos directly depends on the age of the surrogate mother and on the quality of embryos.

Success rate also depends on the age of the biological mother. This method is very effective, even the most effective in reproduction medicine, when the surrogate mother is healthy and donor oocytes are of good quality. Success rate is 70-75%.

What are surrogate mother requirements?

A woman who agrees to become a surrogate mother may do so on compassionate grounds. For example, a close relative or a friend. However for many women this is a financially motivated decision.

In accordance with Russian legislation, a potential surrogate mother should not be older than 35 years old and she should have, at least, one child of her own. Also according to legislation, a potential surrogate mother should sign a consent for registering a baby with the names of its biological parents. After that, Civil Registry Office will issue a birth certificate to the couple.

A couple and a potential surrogate mother sign an agreement. Biological mother must be present at the birth and take care of the baby. Taking special hormonal medications, she can even breast feed the baby.

The cost of surrogacy is high and to be sure that a potential surrogate mother meets all the requirements, our specialists provide complete medical examination. Potential surrogate mothers have a consultation with the psychiatrist. The results of the examination are given to the infertile couple. When a surrogate mother is selected, agreements and other documents are signed by the parties. The first step is the IVF cycle. When a child is born, specialists of our clinic will assist in preparing all the documents to get a birth certificate with the names of biological parents.

Indications for in vitro fertilization surrogacy are the following:

  • Uterine malformation or the neck deformation as a result of failure of development or acquired as a consequence of the disease; irreparable uterine synechiae;
  • Uterine malformation or the neck deformation as a result of failure of development or acquired as a consequence of the disease; irreparable uterine synechiae;
  • Genital or extra genital pathologies making pregnancy impossible; pathological contraindications to pregnancy;
  • Multiple unsuccessful IVF attempts when the transfer of embryos of good quality did not lead to pregnancy.

Surrogacy: cost and legal aspects

On January 2012 a new act On the Fundamentals of Public Health Protection in the Russian Federation came into force. This act determines a group of people who are entitled for this reproductive technology as well as criteria applied to potential surrogate mothers.

In accordance with legislation, biological parents can not choose the sex of their baby. Today not only officially married couples can choose surrogacy but also couples in union and single women.

Requirements to women who will carry a pregnancy for a fee and who are not genetically related to the baby are defined by the legislation. Besides, the legislation allows that a potential surrogate mother can be an oocyte donor which means she will be be both surrogate and biological mother.

Medical examinations and tests for potential surrogate mothers:

  • photofluorogram
  • physician’s conclusion confirming that a woman is able to carry a pregnancy and that she does not have any contraindications. This conclusion as well as photofluorogram is valid for 1 year;
  • consultation with psychiatrist;
  • blood group and Rh factor;
  • HIV, RW and HbsAg, antibodies to hepatitis C tests. Results are valid for 3 months;
  • complete blood count. Valid for a month;
  • hemostasiogram;
  • common urine test;
  • pap test;
  • endometrial cytology by scraping;
  • biochemical blood test;
  • gynecologic examination;
  • pelvic ultrasound;
  • cervical/vaginal cytology, test for urethral gonorrhea, mycoplasma and ureaplasma, CMV and HSV, chlamydia infection;
  • Antibodies to toxoplasma and hepatitis, rubella and CMV

Medical examinations for a couple:

  • photofluorogram;
  • HIV, RW and HbsAg, antibodies to hepatitis C tests. Results are valid for 3 months;
  • blood group and Rh factor.

Medical examination for a biological mother before IVF:

  • therapeutic examination and conclusion about general health;
  • gynecological examination;
  • pelvic ultrasound;
  • common blood count including prothrombin time test;
  • common urine test;
  • pap test and endometrial cytology by scraping;
  • thyroid and mammary gland ultrasound;
  • cervical smear test;
  • Blood hormones: LH and FSH, estradiol and prolactine, TSH and T4 free;
  • ССА125 for repeated IVF.

Medical examination of the father:

  • Spermogram.

Step-to-step guide for surrogacy:

  1. The first step is synchronization of menstrual cycles of the biological and surrogate mother. To do this, GnRH antagonists and oral contraceptives are used;
  2. The next step is endometrial preparation of a woman who will carry a pregnancy. Estrogen and progesterone are used;
  3. Biological mother undergoes ovulation stimulation with gonadotropins;
  4. Then a biological mother undergoes ovarian puncture, oocytes are retrieved and then fertilized with her husband’s sperm or donor sperm;
  5. Embryos are transferred into the uterus of the woman who will carry a pregnancy.

Please, be aware that you pay for the treatment and the fee for the surrogate mother separately. The fee for the surrogate mother is defined by the terms of the agreement and it does not affect the cost of the treatment.
